Job Title: Care Specialist / Health & Wellness Advisor
Location: New York, NY 10004 (Hybrid)
Shift: Monday – Friday | 9:00 AM – 5:00 PM
Contract Duration: Approx. 2 Months
Pay Range: $25 – $30/hr
Key Responsibilities:
• Conduct telephonic outreach to assess members’ health and psychosocial needs
• Develop individualized care plans and coordinate follow-up services
• Support HIV-positive and high-risk members with care coordination and medication adherence
• Promote access to healthcare, behavioral health, and community support services
• Provide health education using motivational interviewing techniques
• Document care management activities accurately and in compliance with regulations
• Collaborate with care managers, providers, and community partners
• Participate in outreach initiatives, case conferences, and quality improvement activities
Required Qualifications:
• High School Diploma or GED
• 1+ year of healthcare or behavioral health experience
• Strong communication and telephonic customer service skills
• Ability to manage outreach activities and maintain accurate documentation
• Experience in care coordination or community outreach
Preferred Skills:
• HIV care experience
• Community outreach experience
• Telephonic care management background
• Experience working with vulnerable populations
• Motivational interviewing and care coordination skills
