Job Description:

• Degree: Doctor of Pharmacy (Pharm.D.).
• Education: Graduate from a college or university accredited by the Accreditation Council on
• Pharmaceutical Education (ACPE).
• Internship/Residency: Completion of a clinical pharmacy residency or fellowship accredited by the
• American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
• Licensure: Current, full, active, and unrestricted license to practice as a Pharmacist.
• BLS: Each HCW shall possess and maintain current certification in one of the following: American Heart Association Basic Life Support (BLS) or American Red Cross Basic Life Support (BLS) for Healthcare Providers or American Heart Association or American Red Cross Healthcare Provider Course. HCWs who do not hold current certification must acquire certification prior to initiating contract performance. Web based classes do not meet these standards.

Experience:

• Possess a graduate certificate within the preceding year from an accredited pharmacy residency program or Possess a minimum of 12 months of experience within the preceding 18 months as a Clinical Pharmacist performing direct patient care and clinical activities in an inpatient, outpatient, or ambulatory care pharmacy setting or have held clinical privileges as a Clinical Pharmacist within the preceding 36 months in a MTF. Must have specific training or experience in the area of pain management services.
