Job Title: Social Worker

Position: Full-Time

Location: Bethany Home Sioux Falls

Salary: $57,000 – $62,000 annually (based on experience)

At Bethany Home Sioux Falls, we are caring people caring for people —and we’re looking for a compassionate Social Worker to join our team.

In this role, you will work closely with residents, families, and the interdisciplinary team to support each resident’s psychosocial, emotional, and social well-being. You’ll play a key role in care planning, advocacy, and helping residents maintain dignity, independence, and quality of life.

What You’ll Do:

• Complete social service assessments and contribute to individualized care plans
• Participate in care conferences and collaborate with the interdisciplinary team
• Coordinate admissions, transfers, and discharges in compliance with regulations
• Assist residents and families with emotional support and resource navigation
• Maintain accurate and compliant social service documentation
• Advocate for resident rights and promote dignity, privacy, and independence
• Serve as a liaison between residents, families, staff, and community resources
• Support residents’ social, emotional, and spiritual needs
• Monitor and report changes in resident condition to appropriate team members

What We’re Looking For:

• Bachelor’s or Master’s degree in Social Work
• LSW or LCSW (or ability to obtain licensure)
• Experience in long-term care, healthcare, or discharge planning preferred
• Strong communication, organization, and interpersonal skills
• A team-oriented mindset with a passion for resident-centered care
